Surgical Lip Lift
Surgical lip augmentation was used in the past only in older women, but its technique has evolved so much in recent years that it now produces wonderful results in very young patients, even in their twenties. If one is not yet ready to lie under a knife, lip augmentation with hyaluronic acid or botox, which is only temporary but also spectacular, can offer a solution.
According to America’s leading plastic surgeons, lip augmentation has become so popular in recent years because with this treatment, they can achieve a more youthful and natural look than just with a lip filler, which “is not good for all mouths.” In addition, the filling is only a temporary solution, which over time – at the site of a series of cannula piercings – can cause scarring and, although it can slightly increase the volume of the mouth, even permanently, the effect does not always show well.
In reality, the main reason is to be found in the anatomical features because the lip augmentation with hyaluronic acid fillers shows the least advantage in those with a large distance between the nose and the upper lip. This is because the filling further lengthens this part, as the filler that gets into the lips makes it harder and “pulls” the lips, which makes it even lower. Aesthetically, we consider it ideal to see a small part of the upper teeth when the mouth opens loosely. However, with age, the upper lip stretches, covering the teeth, which is one of the telltale signs of aging. A shorter nose-lip distance, on the other hand, shows the face optically smaller, finer, thus restoring its youthful harmony and sensuality.
The goal is always a proportionate face, but there is no specifically defined nose-to-mouth distance that would be applicable to everyone. Lip augmentation also results in fuller upper lips, which can be adjusted according to the patient’s idea, but the result will always be natural.
There are several types of lip lift technic. The most common is the Bullhorn, also known as the subnasal lift, also known as the bullhorn lip flip. This method takes out a strip of skin from under the nose, in the form of a bullhorn, hence the name of the procedure. After removing the cut skin, the upper lip is lifted to sew the cut together, just below the nose. Thanks to today’s advanced technique, the scar runs below the natural line of the nose, making it almost invisible and not affecting the shape of the nose.

The so-called Italian lip lift is a variant of Bullhorn lip surgery. Here, the incision is not made under the entire nose, but the surgeon makes only two shorter incisions under the nasal ligaments. This procedure does not raise the center of the upper lip, only the corners of the mouth. Italian lip surgery is recommended for those who only need a small lift. Scarring is even more negligible here due to fewer cuts and the result is more natural.
Another fine method is heel lip lifting. This is mainly used in older patients who want to lift the hanging corners of their mouths. This doctor removes two triangular small pieces of skin from the “turned” corners of the mouth – due to sagging skin – so that the edges of the mouth curl nicely upwards after the operation.
Lip augmentation called “V from Y” offers a great solution for patients who don’t have a long nose-to-mouth distance but narrow lips. Here, the surgeon drops a V-shaped incision on the inside of the lip, which is sewn together into an Ipsilon shape, making the lips fuller. Here the healing time is longer (4-6 weeks) and the change is very noticeable.
In case of Gullwing lip lift, as with the Bullhorn, we also achieve a fuller upper lip by cutting out a strip of skin. The cut in this method runs not under the nose but over the mouth, however, the scarring is more visible, so it is not as popular.
Lip augmentation is a 30-40 minute procedure performed under local anesthesia. The mouth will become swollen for at least a week after surgery and the skin may stretch for another 6-8 weeks. If anyone is interested in this procedure but does not dare to cut into it yet, we recommend lip augmentation with botox. Here, we paralyze the top layer of muscle around the mouth temporarily, thereby “curling up” the upper lips a bit. This method is especially recommended for those whose lips are so thin that they disappear when they smile. The circular muscle around the mouth is very thick and only the upper layer is affected by the botox, so there is no need to be afraid that our lips will be completely paralyzed and will salivate. How much botox will be needed depends on the patient, but it takes approx. Injecting 2-3 units achieves the desired increase and the effect can last for 3-6 months.
